WHY CHOOSE CASE MANAGEMENT

“What I’ve found the most in all my years of doing this work is that everyone is afraid of those first initial months back home.” – Jennifer Angier, Vice President of Addiction Services, MBH

In early recovery, the most important thing is uncovering the areas that you haven’t walked through yet and applying the techniques you learned during treatment. There are going to be things that surprise you, things that are triggers, things you didn’t even know weren’t working for you until you stumble upon them.

That’s where your Meadows Case Manager comes in. We want to make sure the conversations started in treatment follow you home. We’ve been here before and can serve as a guide, helping you create a safe space to recognize weak spots and address them in order to sustain the life you’ve been working so hard to achieve.

What to Expect

SUPPORT

Your Meadows Case Manager is available 24-7 as a support — whether something is good or bad or you just need to talk. Our goal is simply to make the transition as smooth as possible. We are here to help guide and create appropriate boundaries, to celebrate with you when you achieve goals, and to offer help or a reset when you hit a bump in the road.

We’ll never intrude. A case manager won’t be showing up in your home or life uninvited. Everything about this experience is an invitation into setting up the best results you can possibly have as you continue working to create the life you want.

We are so thankful that we chose to go with a case manager after our daughter went through treatment at The Meadows. Our case manager went above and beyond to make sure our daughter as well as our family had extra support and wise advice in helping make decisions during every stage and level of her recovery.  – Case Management Patient’s Parent

A MESSAGE TO LOVED ONES

Meadows Case Management isn’t just for the patient, it’s for family as well. You’ve already made a sizeable investment in your loved one’s recovery — the investment of your time, of your spirit, of everything that you and your family have put into this experience. And you chose The Meadows for a reason. Meadows Case Management allows you protect that investment you’ve already made. Progress has already been made. We want to help bring that back home.
You can’t expect the changes that have occurred to just tuck neatly back into the family. We can help foster healthy communication between family members so you can avoid falling back into old patterns. We’re not here to change what is already working for you as a family, but with a Meadows Case Manager in place, you can return to your intended role of spouse, parent, or partner and not feel like you need to take on the role of warden or caregiver. We don’t guarantee everything will always go smoothly – but we’ll be there to help and keep everyone moving in the same direction.
