Case Management will look different for every person.

The level of need and length of involvement varies depending on each unique case. The common thread is how you benefit from our expertise, experience, and ability to help you anticipate and overcome any barriers to your goals and your recovery.

Customizable Support

For you and your family, Meadows Case Management may look like any of the following scenarios:

  • One-on-one transportation/companion support for the immediate transition home
  • Intensive help during the first weeks, months, or year of recovery
  • Ongoing help integrating the work you did in treatment over the course of a year
  • 24-7 assistance for patient and family

The building blocks of Meadows Case Management

COMMUNICATION

In early recovery, the most important thing is uncovering the areas that you haven’t walked through yet and successfully applying the techniques you learned during treatment into everyday life, so plan to be in touch with your Meadows Case Manager daily. They can serve as a sounding board or a safe space, providing perspective or a reality check. They can also be a bridge between you and loved ones as you transition into healthier roles.

EDUCATION

Learning doesn’t end when you’ve completed a treatment program. Knowledge is power. That’s why we provide ongoing education for the patient and family members in the form of psychoeducation sessions specific to your situation led by our Meadows Senior Fellows or key MBH staff. You’ll also get regular individual and family sessions with Meadows Case Management leadership and the opportunity to attend workshops at Rio Retreat Center.

RESOURCES

It’s vitally important to continue the work you’ve begun, but you may not know exactly how to do that. We can help with practical resources. This may include reconnecting with an existing therapist or establishing new therapeutic alliances with providers in your area, locating 12-Step meetings or other community support groups, and finding resources for family members and loved ones as well. You also get access to our staff medical professionals.

SUPPORT

While much of our support is focused on your recovery, that also includes helping you live the life you imagine. That may include uncovering career goals or helping you pursue academic, physical, or spiritual interests. We can even go into situations with you, providing physical support that is invisible to the rest of the world. Of course, we also offer crisis response so that if a relapse happens, there is already a plan in place to get you back on course.

Highlights of Meadows Case Management include:

• Daily Case Manager contact
• Practical help with details of day-to-day routine and scheduling
• Weekly virtual Brain Center work
• Monthly Meadows Senior Fellow or staff members webinars/private session
• Weekly session with Vice President of Addiction Services
• Weekly family psychoeducational sessions with Vice President of Addiction Services
• Availability of Dr. Kevin McCauley/Jennifer Angier to provide family education sessions (in person or virtually)
• Two on-site workshops at MBH’s Rio Retreat Center (including lodging and meals)
• Monthly and/or emergency face-to-face meeting

Since Meadows Case Management services are so individualized, rates are determined by the specific services needed and the length of time they are required.
